One of the pillars of health care is portability, i.e. care anywhere in Canada. The practical realities are a bit different but it's certainly easy, and desirable to some health authorities, for patients to travel.

I believe the original deal struck between the Feds and Provinces was a 50/50 split in funding, the Feds $ went directly to health care.
I forget the exact figure but the Feds current share has been whittled down to something like 12-19% and that isn't even earmarked specifically for health care, it can be folded into general revenue.
